But once the export is done Premiere Pro CC has produced 4 files:

Or you can change Format from "H.264 Bluray" to "H.264" and uncheck "Export audio" and the resulting file will be a .m4v. If you check the audio, the output will be MP4 format cause as far as Premiere is concerned, m4v is video only.
